POSITION TITLE: Senior and In-Home Services Counselor and Case Manager
LOCATION/DEPT: City Mission
REPORTS TO (TITLE): Senior and In-Home Services Program Manager
FLSA CATEGORY: Non-Exempt
STATUS TYPE: Full-Time
To assist in accomplishing the mission of The Salvation Army through the coordination of all mission, corps and social services within the City of Chicago.
Provide counseling, support groups, gap filling and other case management services to individuals 55 and older with primary responsibility for raising their minor grandchildren or other relative minors through the Grandparents or Older Relatives Raising Children.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides assessments to determine needs of families and provide them with the appropriate services.
Provides, within agency policies, programs, and procedures, direct counseling and casework services to individuals and families.
Conducts educational support groups that related to parenting and family issues.
Assists families with GAP Filling Funds that complement the care provided by the older relative.
Works collaboratively with partners as well as developing new partnerships within The Salvation Army and the community to provide resources for participants.
Maintain, in a timely manner, required documentation of participant's involvement including but not limited to assessments, demographic data, case notes, closing summaries, and outcomes data collection.
Obtain appropriate releases of information.
Conducts outreach activities to bring awareness and participation in the program.
In consultation with the Senior and In-Home Services Program Manager initiates contact with the Illinois Department of Children and Family Services as a mandated reporter of suspected child abuse or neglect.
In consultation with the Senior & In-Home Services Program Manager, initiates contact with the Elder Abuse Hotline as a mandated reporter of suspected elder abuse.
Participate in weekly supervision and case consultation with Senior & In-Home Services Program Manager. Seek consultation from the Program manager as needs arise.
Attends Senior & In-Home Services staff meetings as scheduled.
Maintain awareness of applicable liability issues and other applicable legal issues, and potential effect on clients and/or the agency.
Abide by the ethical and culturally competent standards as outlined by the National Association of Social Workers and The Salvation Army.
All other duties as assigned.
Awareness of agency policies and procedures.
Attend agency general staff meetings and professional trainings as applicable.
Perform special administrative assignments or duties as may be delegated.
Collaborate with other Salvation Army units and personnel to successfully maintain and achieve policies and procedures, goals and objectives, and outcome-based measures related to the Hope-Strong philosophy.
Understands, and can effectively work under, the Mission of The Salvation Army.
Attend meetings with community partners, as needed.
Participate in community events as part of public relations and marketing of the Grandparents or Older Relative Raising Children Program.
Develop partnerships and resources with other service providers.
Other Duties as assigned.
Minimum of a Masters Degree in Social Work or related field. LSW is required, with an LCSW preferred.
Experience with counseling individuals and families.
Experience with conducting groups.
Experience working with seniors preferred.
Knowledge of the components of program services and the resources available.
The ability to effectively integrate program services with organizational concepts.
Demonstrated ability to use initiative, work independently, and be a self-starter.
An interpersonal style that promotes collaborative and effective team relationships.
Flexibility in accepting assignments with various types of clients and environments.
Must be sensitive to and work appropriately with culturally and socio-economically diverse participant population.
Warmth and a strong commitment to helping people, along with acceptance of varying lifestyles.
Willingness to work with seniors in inner-city neighborhoods.
Excellent communication (verbal and written) and interpersonal skills.
An ability to use technology to conduct interviews, counseling sessions and group meetings.
An ability to work with confidential material.
Driver's license, MVR clearance and a working, appropriately insured automobile.
