Case Managers (acute care)
The Case Managers at Dartmouth-Hitchcock are Registered Nurses who are tasked with the evaluation and implementation of health care plans for individual patients in the acute hospital. As a member of the multi-disciplinary team, Case Managers are responsible for providing effective and efficient medical care by triaging, coordinating and facilitating the clinical management for an assigned population. Our Case Managers work collaboratively with inpatient departments that have patients ready for discharge and assist with patient and family education, future clinical planning needs as well as connecting patients and families with available community resources.
As a Case Manager you will also develop, monitor and reinforce care standards and protocols designed to enhance quality and promote cost-effective utilization of healthcare resources.
Graduate from an accredited Nursing Program required.
Bachelor's degree (or matriculated into a bachelor’s degree program with completion within a period of time agreed upon with the department at the time of hire) with 3 years of clinical experience required.
Recent acute care hospital experience preferred.
Desired qualities and behaviors include being a collaborative team player with strong communication skills, use of a systems approach in planning, problem solving and decision making, creativity, innovation, risk taking, autonomy, flexibility, receptiveness to change and a commitment to professional growth.
Computer skills required.

MinimumQualifications for MSW:
Master’s Degree in Social Work (MSW) required.
Social work experience in an acute, home care, or ambulatory setting preferred.
Strong organizational skills, verbal and written communications skills.
Sound decision making and judgment.

Proven ability to deal with conflict resolution.
Comfortable with basic computer skills.
MinimumQualifications for Continuing Care Manager:
Master’s Degree in Social Work (MSW) with 5 years of clinical experience required.
Critical thinking skills, strong leadership and communication abilities, the ability to be a self-starter, negotiate, and handle conflict resolution desired.
Understanding of and comfort using computers desired.
Collaborative team player, use of systems approach in planning, problem solving and decision making, creativity, innovation, risk taking, autonomy, flexibility, receptiveness to change and a commitment to professional growth desired.
