The Inpatient Acute Care Coordinator is a Registered Nurse who coordinates care for a group of patients admitted in Inpatient Acute Units. In addition to generalist preparation in nursing with enhanced clinical skills, the CC is primarily unit based and is responsible for a specified cohort of patients with complex health needs. The Inpatient Acute Care Coordinator demonstrates expertise in the application of the nursing process, assessment, plan, intervention and evaluation since patient admission until the patient discharge. He/she also provides transition of care to patients identified high risk. Skillfully and effectively applies all aspects of the nursing process and clinical care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and disease prevention practices, management of acute and chronic illnesses, and attainment of appropriate lengths of stay and most effective level of care. He/she coordinates the care of patients admitted, leads the discharge huddles and determinates potential patient at risk during discharge and determinates the patient needs for follow up calls until their first post discharge appointment. He/she works in collaboration with the interdisciplinary team to assure that the patients care plan goals and needs are approached out, revised as needed and modified between team meetings. He/she reports directly to the Nurse Manager. The CC for Inpatient Acute Units interprets the philosophy and goals of Nursing Service; functions within establishes guidelines and demonstrates the ability to work effectively and develop effective relationship with patient/ relatives and other professionals and supportive personnel who provide client care. He/she serves as a liaison between nursing staff, and the interdisciplinary team. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The Veteran Caribbean Healthcare System (VACHS) is a Level 1a major interdisciplinary teaching facility with service-lines serving medical, surgical, and neurological inpatients and outpatients, with a Level Ill Emergency Department, and multiple community-based outpatient clinics (CBOC). There is a large outpatient psychiatric population, with admissions for medical co-morbidities. The Southeastern Blind Rehabilitation Center provides services in a residential setting.
