The UCSD Mother Child & Adolescent HIV Program provides comprehensive and integrated HIV care, social services and clinical research to women, children and youth ages 13-24. A multidisciplinary team of HIV specialists provides medical care, treatment, health education, nutrition education, mental health counseling, case management, and clinical research studies.
Under close supervision, the case manager performs a full range of social work duties and responsibilities. Primary responsibilities include conducting comprehensive assessments and formulating care plans, providing crisis intervention, intensive case management, and coordination of services for women and youth with HIV, as well as their partners, children and families. Participate in multidisciplinary patient care conferences and coordinate implementation of interdisciplinary plans. Through individual, couple, or group interventions, assist individuals in adjusting to the physical and emotional challenges associated with HIV. Assist patients in accessing health care benefit programs and community resources. Ensure that all treatment plans and delivery comply and adhere to federal, state, and local social work code; deliver ethical, nonjudgmental and nondiscriminatory care, with sensitivity to cultural, religious, sexual health, developmental, and/or gender-responsive needs; and preserving/protecting patient autonomy, dignity, and rights in alignment with the Patient Bill of Rights. Professional and community activities include participating in program and community committees; developing and/or coordinating special projects and/or programs for patients, supporting grant opportunities. Administrative responsibilities include participating in quality assurance and program evaluation activities, and maintaining departmental statistics. Other professional activities include participating in community planning, developing and/or coordinating special activities for patients, maintaining clinical files, and generating departmental reports.
A Master's degree from an accredited school of social work.
Valid CA driver's license and acceptable driving record.
Demonstrated skill in social work assessment of and interventions with individuals and families impacted by chronic disease and/or traumatic experiences. Strong crisis intervention skills.
Experience working with children, adolescents and adults in a medical or social service setting.
Demonstrated experience with social work interventions, safety assessments and required documentation in cases of suspected child abuse/neglect, interpersonal/intimate partner violence, elder/dependent adult abuse, mental health crises, and substance abuse.
Demonstrated experience integrating theory and practice in the formulation of psychosocial assessment and interdisciplinary treatment plans, the implementation of care plans and the modification of treatment based on outcomes.
Demonstrated experience conceptualizing and writing clear, concise, appropriate psychosocial assessments, care plans, chart notes, and progress reports in social work and medical records.
Demonstrated experience thinking creatively and efficiently, to assess, analyze and solve problems.
Demonstrated ability to deliver care in an ethical, nonjudgmental, culturally competent, and nondiscriminatory manner.
Ability to respect confidentiality and professional judgment to interpret, clarify, and report information to patients, family members, and staff.
Demonstrated ability to interact as a member of an interdisciplinary team.
Demonstrated experience communicating effectively and concisely in writing and verbally in formal and informal settings.
