Job Summary:

The Department of Medicine/STAR Program at SUNY Downstate Health Sciences University is seeking a full-time Case Manager / Social Worker Assistant. The successful candidate will: 

• Assist patient in obtaining needed services such as insurance benefits, entitlements, housing, transportation, vocational assistance and referrals to community-based agencies for other services.
• Complete letters, forms and other paperwork for patients, based on need and eligibility.
• Document all patient contact and activities on behalf of the patient in the EMR.
• Work collaboratively with case management, medical and other interdisciplinary staff and coordinate patient care, maintaining communication through patient huddles, EMR messaging and verbal communication.
• Assist patients with enrollment into Medicaid, uninsured care programs, and enrollment into the healthcare marketplace.
• Assist patients in completing paperwork for cap on charges and sliding fee scale when applicable. Ensure all patients bring in income verification annually, in accordance with grant funding requirements.
• Participate in performance improvement process.
• Participate in health center outreach and patient retention activities, including occasional weekends.
• Adhere to University Hospital of Brooklyn’s Principles of Behavior.
• Conduct Mental Health and Substance use screening.

Required Qualifications:

• Bachelor’s Degree in Social Work or related field.
• 2+ years of experience providing case management services in a health care or social service agency.
• Excellent organizational and communication skills required.
• Certified Application Counselor (CAC) credential required, but may be obtain upon hire.

Preferred Qualifications:

• Experience with underserved populations such as people living with HIV/HCV, LGBTQ, and those with mental health and substance use disorders strongly preferred.
• Bilingual preferred.

Work Schedule:

Monday to Friday; Variable Hours (*1-2 evenings required)
