Case Manager Assistant
Major Duties: - Assists in completing in-depth reports, such as assessments and person-centered plans, including recommendations, activities and interventions.- Assists with scheduling and facilitating the interdisciplinary assessment and planning process for a select group of clients, including follow-up team meetings and periodic reviews.- Assists the Case Manager in visiting clients and taking clients to appointments. Monitors services as needed and links to community resources.- Facilitates connection to community and meaningful activities.- Provides clerical support to the Case Managers including filing, copying and mailing of documents.Minimum Qualifications: - Bachelor's degree or higher in a related field from an accredited institution.-
