This is a Telephonic RN Case Manager position that will be supporting the Enhanced Family Support (EFS) Program in the one nurse model - providing case management support from fertility, maternity through post-partum.
You’ll enjoy the flexibility to work remotely* from anywhere within the U.S. as you take on some tough challenges.
This position is a full time/40 hours per week. This role is required to work one shift from 10:00am to 7:00pm (in the employee’s time zone). All other days are standard work hours.
Primary Responsibilities:
- Make outbound calls to assess members' current health status
- Identify gaps or barriers in treatment plans
- Provide patient education to assist with self-management
- Make referrals to outside sources
- Provide a complete continuum of quality care through close communication with members via on-phone interaction and a digital platform chat feature
- Support members with condition education, medication reviews and connections to resources
Required Qualifications:
- Current, unrestricted RN license in the state of residence
- Compact License and/or ability to obtain additional state nursing licenses (application and fee paid for by UHG)
- 3+ years of nursing (RN) experience in a fertility clinic where working with fertility patients was the focus of the role
- 2+ years of nursing (RN) maternity/OB experience
- Experience managing/supporting patients with co-morbidities (hypertension, cardiovascular disease, diabetes, edema/renal disease, genitourinary infection, general infection, thyroid disease, anemia)
- Strong clinical knowledge in the field of infertility with the ability to provide education to member who are seeking infertility treatment
- Intermediate proficiency, including the ability to type and talk at the same time and to navigate within Microsoft Office
- Access to high-speed internet from home
- Dedicated workspace in home for home office
- Ability to work one night per week until 7:00pm (in the employee’s time zone)
Preferred Qualifications:
- BSN
- Certified Case Manager (CCM)
- Background in managed care
- Knowledge of 3rd party reproduction
- Willingness to obtain CCM within 30 months of employment
- Case management experience
- Experience or exposure to discharge planning
- Experience in utilization review, concurrent review or risk management
- Experience in a telephonic role

Colorado, Connecticut, Nevada, or New York City Residents Only: The salary range for Colorado residents is $54,400 to $97,000. The salary range for Connecticut/Nevada/New York City residents is $60,000 to $106,700.
