Anesthesia, Gynecology, Orthopedic, Orthopedic Foot & Ankle, Pain Management, Plastic Surgery, Podiatry, General and Spinal Surgery.
JOB SUMARY
Facilitates progress through the continuum of care, from admission through discharge. Provides feedback to payors on patient's clinical status and secures approvals for additional days of stay. Monitors quality standards as determined by the Performance Improvement Plan and the Medical Staff Quality Indicators. Evaluates patients for discharge planning needs and appropriately directs the patient's care. Monitors resource consumption for most effective use and cost control. Identifies teaching and psychosocial concerns for the best patient outcomes. Assists in aggregating data for quality trending and quality improvement. Assists in Infection Control surveillance. Functions as a Patient Advocate to assist patients in resolving concerns.

RESPONSIBILITIES
General:
The Case Manager works with all departments to ensure appropriate patient care.
Is able to work effectively with insurance co., Medicare etc in making sure that we are paid appropriately for the patient care services we provide.
Keeps the Chief Financial Officer and CNO aware of any problems that have occurred in any area of patient care.
Works with the physician's office to ensure that we have the correct status for the patient and that the patient received the correct post discharge care.
Serves as a resource to Senior Management.
Participates and reports on Utilization review at all CQI meetings.
Manages LOS appropriately to patient care needs.
Manages avg daily census appropriately to patient care needs.
Reviews all inpatient denials for potential peer to peer appeal.
Attends all USPI case management calls.
Attends Quality calls when available.
Demonstrates ability to prioritize tasks and demonstrates flexibility as job priorities change, willing to take on new work routines/methods.
Communication is clear and concise, free of gossip, respectful, confidential, friendly and courteous.
Required Experience:
Licensure as a Registered Nurse in the State of Pennsylvania
Bachelor's of Nursing required
Demonstrates ability to stand and/or walk at five hours per day
Current BLS Certification
Current ACLS & PALS Certification
Minimum five (5) years acute care experience required
