Job Details
The Inpatient Patient Resource Coordinator works closely with patients, families, and the Inpatient Case Management Care Team to facilitate community connections and access to a range of psychosocial resources within CHA and in the community. The Patient Resource Coordinator performs a wide range of functions that safely, effectively, and efficiently support CHA patients to address their personalized health goals. Responsibilities include working with patients and the inpatient care team to access resources and remove barriers to social support that impede patient health and safety. Effectively builds supportive, short-term, relationships with patients to solve such problems. Efficient use of resources will improve patient experience of care, promote population and community health and wellness, and ensure patient engagement and empowerment.
Qualifications/Requirements
Education/Training: Two years of health science related coursework is required.
Bachelor's degree in Social Work (or equivalent Human Service field) is preferred.
Language proficiency in a second language is highly desired.
Licensure: LSW is preferred not required.
Certifications: Community Health (CHW)worker training or certification (or equivalent) is highly desired, Community Health Education Center (CHEC) training is highly desired, Area Health Education Center (AHEC) training is highly desired
Work Experience:
1-3 years of related work experience is strongly preferred
Experience with relevant community groups and multicultural settings is strongly preferred
Experience with mental health, addiction, frail elders is strongly preferred
