The Behavioral Health Case Manager position is part of the Behavioral Health Services team at the 1930 Market Street Service Center, as well as part of a collaboration with Zuckerberg San Francisco General Hospital (ZSFG) Positive Health Program (Ward 86). This role is responsible for providing case management services to individuals and groups who are Intravenous Drug Users (IVDU) and who are at-risk of HIV and other comorbid diseases, as part of the Health Access Point (HAP) for IVDU providing comprehensive HIV prevention and Harm Reduction services for these individuals. These clients are likely to have substance use disorders and potential co-occurring mental health disorders.
