Case Management is defined in State Statute as providing coordination and access to services required by people experiencing serious and persistent mental illness (SPMI, adults) or serious emotional disturbance (SED children). The clinic has a contract with Hennepin County and various Managed Care Organizations to provide these services to eligible patients who reside in Hennepin County. The scope of the work and the requirements for supervision of the work is found in MN. Statute and in the MN.
