THE POSITION: Under general supervision of a Registered Nurse, Physician Assistant, or Physician, the incumbent is responsible to assist with patient care coordination and utilization review. The incumbent will be responsible to assist with evaluation of patient care data in order to coordinate post-discharge needs of patients.
THE IDEAL CANDIDATE: The ideal candidate for this position will be adaptable, flexible, and possess high-level critical thinking skills. They will be dedicated to providing excellent customer service while interacting with the public, and will able to work in a fast-paced environment. Finally, the ideal candidate will have previous work experience in case management, managed care and/or utilization review, and will possess basic knowledge of Microsoft Excel.
PAYROLL TITLE: Licensed Vocational Nurse
EDUCATIONAL/BILINGUAL INCENTIVE: Incumbents may be eligible for educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.
CERTIFICATION PAY: Incumbents in this classification are eligible for certification pay at a rate of $0.813 per hour based on his or her scheduled work week hours and not to exceed the scheduled work week hours for each qualified certification possessed up to a maximum of five (5) certifications. One of the five certifications may be paid at $2.00 per hours for a National Certification. List provided in the MOA, Exhibit 10 (please see attached list here(Download PDF reader)).
AGENCY/DEPARTMENT: Health Care Agency - Ventura County Medical Center (VCMC)
Licensed Vocational Nurse is represented by the California Nurses' Association (CNA) and is eligible for overtime compensation.
NOTE: Salary placement will be determined according to the current California Nurses' Association (CNA) memorandum of agreement.
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Intermittent, and Extra-Help vacancies for the Health Care Agency. There is currently one (1) Regular vacancy at the Ventura County Medical Center (VCMC).

Examples Of Duties
Duties may include but are not limited to the following:
Performs patient care functions and initiates appropriate nursing interventions as outlined in standards of care and the individual patient care plan in support of professional staff;
Collaborates with registered nurses, physicians, and physician assistants to coordinate post-discharge needs of patients;
Communicates the post-discharge plan to patients, family/caregivers, staff & appropriate community agencies;
Acts as a liaison between in-patient facility & referral facilities/agencies & provides case management to patients referred;
Obtains all authorizations/approvals as needed for outside services for patients/families in accordance with the appropriate health care plan;
Refers patients to the community resources to meet post hospital needs;
Performs basic assessments (data collection) and documents variances;
Depending on assignment, may transcribe physician’s orders, including telephone and/or verbal orders, and take necessary steps to ensure completion;
May participate in developing nursing care plans and the maintenance of patients' records; and
Performs other related duties as assigned.

Typical Qualifications
These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.
EDUCATION, TRAINING, AND EXPERIENCE: Requires one (1) year of full-time licensed vocational nursing experience in a health care setting that includes at least six (6) months of full-time licensed vocational nursing experience comparable to that obtained at the Ventura County Medical Center in the Health Care Agency.
NECESSARY SPECIAL REQUIREMENTS:
Must possess and maintain a current valid license as a Vocational Nurse issued by the State of California
DESIRED:
Previous experience in case management or utilization review
Previous professional experience using Microsoft Excel
KNOWLEDGE, SKILLS, AND ABILITIES: Working to thorough knowledge of: the techniques and practices of nursing; medical terminology, routines, and equipment; the role in abuse/neglect reporting; and confidentiality policies.
Skill in: the transcription of physician’s orders, including telephone/verbal orders and the completion of those orders.
Ability to: follow agency and unit policies and instructions as given; establish and maintain the confidence of patients; maintain confidentiality of patient/client information; work effectively in different healthcare and/or hospital departments or shifts as required; establish and maintain effective interpersonal relations with other staff members, patients, and their families; communicate effectively both orally and in writing.
