Responsibilities
The purpose of the Case Manager Assistant is to support and under supervision of the Clinical Case Manager to coordinate referrals and implement in facilitating the discharge plan developed and identified by the Clinical Case Manager. This position efficiently and effectively coordinating movement through the continuum of care to ensure timely discharge by effectively coordination of necessary durable medical equipment (DME), home care services, home health nurse/aide, therapies, hospice, home infusion, enterals and teaches. The CMA will compile documentation from the patient's chart through Epic to produce a discharge packet for all patients discharging to a SNF/Acute/Rehab facility this packet will be delivered to the patients floor before the arrival of the ambulance. The CMA will monitor all primary and secondary Medicare in-patients insuring they are delivered the Important Message from Medicare before discharge. The CMA will
reach hospital authorizations approvals/denials and requests for additional medical documentation. Verify correct level of care in the Clinical Case Management module and Epic Auth Certification tab.
Required Qualifications
Minimum two years (2) experience in acute or post-acute discharge planning or case management, experience in a large academic healthcare setting.
