The Case Management and Care Transitions Professional Development Specialist I reports to the Director of Case Management and Care Transitions or their designee. Under indirect supervision, the Case Management and Care Transitions Professional Development Specialist I consults with the Case Management and Care Transitions Leadership, ICCE, organizational leadership and interprofessional team as well as the CMO to develop, implement, and evaluate evidence-based, quality and educational programs and activities that promote professional competency and role performance for Nursing and Social Workers, interprofessional and non-licensed clinical staff. The position will assist in the implementation of designated centralized components of education. Education will consist of 1.) centralized orientation 2) annual competence assessment 3) practice orientation and preceptor/buddy development 4) required training 5) ongoing staff development 6) hospital wide affiliations 7) performance improvement.
Hours per week: 40
Scheduled Work Hours/Shift: Monday-Friday. 8:00a-4:30p.

Minimum Training and Education: Bachelor’s degree in nursing from an accredited school of nursing and a minimum of five years of applied Case Management experience as an RN/Case Manager or Master’s Degree in Social Work is required. For nursing candidates, a Master’s degree is preferred. Work experience in an education role (clinical education, professional development, academic faculty) is preferred. Knowledge of Nursing and Social Work Professional Development, adult learning theory, evidence-based practice and measurement of outcomes is preferred. An ability to establish collaborative working relationships with diverse groups and individuals, medical staff, and other health care providers is required.
Required Licensure, Certifications, Registrations: Current South Carolina licensure as a social work in the state of South Carolina or as a registered nurse or compact state required. Certifications in Case Management (ACM, CCM) required within 6 months of hire.
