SUMMARY OF POSITION:

The primary purpose of the Lead Case Manager position is to serve as a resource to the case managers so they can better meet patient needs. The person in this position will assist in the training of new case managers, ensure client files are compliant with the state standards, and serve as a backup when case managers are not available to address client needs. The person in this position will also maintain a small caseload of persons living with HIV.

MINIMUM QUALIFICATION(S):

Bachelor Degree 

Three (3) years of related experience 

Basic Life Support (BLS) 

Must have valid driver's license and reliable transportation.

PREFERRED QUALIFICATION(S):

Previous experience with HIV patient population preferred. 

Bilingual (Spanish/English) preferred.
