The role you’ll contribute:
Attend in-service programs of continuing education as appropriate for job performance.
Be flexible in accepting job assignments displaying good prioritization skills prioritize and organize time and functions within standards.
Respond to resource requests for case management assistance from physician offices and other community agencies and/or individuals.
Incorporates patient, physician, customer needs and concerns into decision-making and organizational action.
Arrange transportation for patient discharges.
Order Durable Medical Equipment as indicated.
Transmit medical record information to nursing homes, rehab facilities, LTAC, home health agencies and hospice organizations for purposes of patient placement and discharge.
Contact various community agencies to obtain available resource information for use by the case manager and/or social worker.
Communicate with nursing units on status and timing of patient discharges and facilitate completion of patient discharge information as indicated.
Meets with the patient and family as needed in order to carry out assigned task.
Assists in ordering of department supplies and in providing phone coverage as assigned.
Assist patients with making follow up physician, treatment appointments as assigned.
Deliver the CMS 2nd Important Message letter and process according to policy.
Acquire/track face to face forms.
Perform follow up phone calls to patients as assigned.
Documents in the patient’s medical record timely and per regulatory requirements.
Cross-trains with the Intake Planner to provide assistance and coverage when necessary.
Attend and participate constructively in department staff meeting.
Forwards to appropriate staff member, requests for clinical reviews left on department voice mail.
Participate in department PI activities, including preparation of various reports out of existing programs.
Assists with data collection and reporting as assigned.
Works collaboratively with staff to move patient efficiently through hospital continuum of care.
Assists in tracking and documentation of avoidable days.
Identifies cases with exceptional discharge delays and collaborates with case manager and Manager/Administrative Director for solution and direction.
Provides conscientious commitment to assigned work, work schedule, honesty, teamwork, assisting others to bring about positive outcomes.
Recognizes and manages risks, resolves issues and problems as soon as they arise, reports and documents adverse events and reportable conditions.
Verbally and behaviorally supports Shawnee Mission Health’s mission and values. Develop a helpful attitude and purpose towards customers,
Demonstrate proficiency in use of the electronic medical record and programs specific to the CMA role.
Rotate weekend and holiday on-site coverage as assigned.

The value you’ll bring to the team:
The Case Management Assistant (CMA) will provide support and assistance to the MSW Social Workers and Case Managers in the completion of discharge planning responsibilities including, but not limited to, ordering DME, arranging transportation and coordinating with nursing units. The CMA documents in the patient’s medical record timely and per regulatory requirements. Documentation may include entries regarding reimbursement data as directed. The CMA cross-trains and assists the Intake Planner as needed. The CMA will be flexible in accepting job assignments while displaying good organizational and prioritization skills. This position requires independent decision making and functioning. Provides conscientious commitment to assigned work, work schedule, honesty, teamwork, and assisting others to bring about positive outcomes. Weekend and holiday rotation is scheduled based on the needs of the department and hospital. The Case Management Assistant will work from referrals from Care Coordination staff. The CMA will respond to resource requests for social work or case management assistance from physician offices and other community agencies.
Qualifications

Bachelor’s Degree
1 Year of Position-Related Experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Kansas Licensed Bachelor Social Worker OR
Kansas Licensed Practical Nurse OR
Licensed in a Healthcare Related Field
