Position Summary
Reporting to the Senior Clinical Case Manager, the Clinical Case Manager is responsible for high-level assessment and care planning for students with chronic and acute complex medical conditions. Ensures a coordinated, collaborative approach to patient care, where the patient???s care plan demonstrates a sharing of information and unity of effort amongst clinicians. Works as part of an interdepartmental team to ensure the delivery of high-quality, effective patient care. Meets the patient???s needs efficiently and expeditiously by continuously improving the patient???s experience and helping to ensure institutional standards of high-quality, effective patient care. Through broad knowledge of clinical care and systems management; the Case Manager predicts, facilitates, and evaluates the movement of the patient through the continuum of care. The position will coordinate referrals to resources within the community to further address the medical, mental health, behavioral health, and social needs of the patients.
Responsibilities
In collaboration with members of the health care team, identifies students with both chronic and acute complex medical conditions, coordinating care as needed. Manage referrals to appropriate levels of care including on-and off-campus providers. Works with students in forming a primary care provider relationship to ensure more coordinated and effective health promotion, prevention, and interventional care. Collaborates with members of Columbia Health to effect optimal patient outcomes. Ensures a warm handoff of Columbia Health patients to campus partners. Acts as patient advocate to ensure that appropriate care and services are delivered as needed. 30%
Reviews clinical case history for incoming students and initiates follow-up communication with entering students, who have identified a significant or chronic medical condition. Gathers information from a number of relevant sources to determine level of services needed for a student with a chronic medical condition and refers to appropriate services. 30%
Reviews a wide range of clinical cases including after-hours and ER reports, hospital inpatients and coordinate after care and referral. 20%
Seeks to expand knowledge of patient-care resources. Maintains knowledge of current research and information related to nursing practice or clinical cases and care management and critically analyzes and applies new information to practice. Collaborates with various stakeholders to identify performance improvement and quality control initiatives. 5%
Collaborates with other departments within and outside Columbia Health to develop a comprehensive and collaborative case management process. 5%
Build relationships with patients that foster early communication of risky situations to mitigate the need for crisis management. Use appropriate screening criteria, knowledge, and clinical judgment, to assess the patient???s current functional level and develop and monitor ongoing care plans. 5%

Minimum Qualifications
A bachelor???s degree and/or its equivalent is required.
Requires current NY State RN or LCSW license.
Minimum of 3-5 years clinical experience in nursing case management, community health nursing, or closely related field.
Requires a high level of independent judgment, initiative, discretion, and ability to problem-solve, use sound judgment, and work with a diverse population.
Must demonstrate ability to communicate effectively in person and in writing with multiple levels of stakeholders, including students, providers, and campus partners.
Ability to establish and maintain effective working relationships with a variety of stakeholders.
Must possess a complete understanding of acuity levels for specific patient populations.
Computer literacy and proficiency.
