The Home Based Primary Care (HBPC) RN Nurse I/Level 3 is a case manager for the Veterans enrolled in their panel. HBPC provides primary care services for Veterans who have a number of chronic medical conditions requiring longitudinal case management by an interdisciplinary team in the home environment. Patient populations include male and female, adult and elderly, frail patients. This is a intermittent position, on call with a varied/intermittent work schedule. Shifts will vary. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Acute Care, Geriatric Care, Nursing Home etc. Clean driving record to legally drive HBPC vehicles to scheduled home care appointments. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: Moderate carrying (15-44 lbs), moderate carrying, 15-44 pounds, straight pulling (.5 hr), pushing (.5 hrs), reaching above shoulder, use of fingers, both hands required, walking (2.5 hrs), standing (5hrs), repeated bending (.5 hrs), both legs required, operation of crane, truck, tractor, or motor vehicle, ability to distinguish basic colors, hearing (aid permitted). ["The RN provides medication management, injections, wound care, anticoagulation, glucose monitoring, blood draws, specimen collection, safety evaluations, assessments, and patient education. The RN is accountable for a primary caseload, for patient assessment, patient/family education, and coordination of the therapeutic regimen in the home setting. The HBPC RN fulfills their role in such a manner to positively reflect on the Veterans Health Administration (VHA). Position may entail other duties as assigned. The incumbent is required to possess a valid driver's license and is expected to independently drive to the residences of patients to complete scheduled appointments. Duties include but not limited to: Provides oversight of the patients' journey through the continuum of care, ensuring that all steps in the process are coordinated, timely and completed- tracking the overall access to services. Provides Veteran centered care and formulate a care plan which incorporates and reflects the Veteran's health care goals. When appropriate, provide services through palliative care model of care. Uses a team approach to provide interdisciplinary case management to Veterans meetings eligibility criteria for HBPC. Provides Veteran, family, and staff education and support. Assesses the Veterans status and identifies priority needs for care throughout the health care continuum. Provides community agencies with patient information on a \"need to know\" basis, according to VA policies and procedures always respecting patients' privacy and confidentiality. Jonathan M. Assesses the complex learning needs of specific patient/family cohorts. Uses appropriate consultants, integrates recommendations into teaching plans and assures appropriate follow up. Maintains HBPC program practice-specific and specialized competencies. Provides leadership using own expertise to assess, maintain, or improve competence of other staff. This may be completed by precepting new nursing employees, student nurses, or providing peer training. Travels to each Veteran's home through HBPC's assigned service as needed/directed by interdisciplinary care team to provide quality care. Develops and implements routine patient monitors. Shares information related to new technology, procedures, medication, trends and practices with co-workers and students. Delivers complex patient care situations in patients' home and serves as a liaison between patients, families, VA staff and community agencies. Coordinates referrals for multiple home services, including but not limited to RN, social work, and physical therapy. Some referrals can be made within the HBPC program and others are made through HCBS for care in the home environment. Maintains knowledge about VA and HCBS program eligibility. Works closely with VA staff and community agencies to resolve resource challenges to meet patient safety and therapeutic needs.
