Case Management is a team process that includes assessment, planning, facilitation and evaluation of the services required to meet the patients needs through the healthcare system. The Case Management Extender will provide programmatic support services for Case Management & Social Work covering patients in UH, Ross, Brain and Spine, Critical Care Tower, East Hospital and Dodd Hall.
