The LHSON Care Case Manager (CCM) will provide consultation, case management services and coordinate care for nursing students who are experiencing mental health and wellness concerns based on referrals received from faculty, staff and students. They liaison with campus, university and community partners to ensure clear communication about appropriate resources to students. They will serve as a member of the Baylor-wide care team that meets twice a month to report on different students and their concerns. Additionally, they will provide limited after hour crisis management services for student crisis situations. This position will also provide student programming for wellbeing initiatives and food insecurity efforts.
A Master's degree and three years of relevant experience are required. Five years of experience are preferred.
Applicants must currently be authorized to work in the United States on a Full-Time basis.
What You Will Do
The CCM will serve as a point person for referrals of nursing student of concern. The CCM will outreach and meet with student to ensure the student has knowledge and adequate access to resources for the unique needs of their situation. The CCM will assist students in navigating the university and resource processes. Tracks and follows up with students for a 2-4 week (typical) period of time after providing them with appropriate resources in order to ensure continuity of care, and to ensure that people get setup with resources and do not end up 'falling through the cracks'.
This position will work closely with the entire LHSON faculty and staff and coordinate outreach efforts to facilitate support for students of concern. They will also serve as a primary contact with our counseling service partner. They will also provide or coordinate educational programs / trainings around mental health, wellness, and support resources. Organize staff / faculty mental health trainings. Coordinate LHSON food insecurity efforts and the LHSON Store.
The CCM will maintain appropriate, accurate, and timely documentation for the students they support. The position will also prepare regular statistical reports of student support.
The CCM will attend LHSON staff / faculty meetings and campus care team meetings. Research national trends to implement safety and support programs for students. Respond to email and calls in a timely manner.
The CCM will provide limited after hour crisis management services for student crisis situations. Coordinate crisis management situations with LHSON leadership, BSWH police, Care Team, and any other campus / community partner needed in the situation.
