JOB DESCRIPTIONCase Manager needed in the Fort Worth, TX service area. Applicants must reside in this community or surrounding communities and hold a TX RN license in good standing to be considered. Our field case managers complete assessments with the members face-to-face in their homes. Applicants with Case Manager experience with another MCO like Molina are preferred, but we will also consider candidates who have a strong background in home health/hospice.   The schedule is Monday - Friday, 8 AM - 5 PM CST.
