JOB DESCRIPTIONWe are looking Case Manager who must reside in MI and will work in remote setting supporting Medicaid Michigan Population.  We are looking for candidates that have experience working with patients that have Substance Abuse disorders, with strong knowledge recovery treatments and connecting to community resources. This role will have you completing assessments, creating care plans, and connect to community resources.  Excellent computer skills and attention to detail are very important to multitask between systems, talk with members on the phone, and enter accurate contact notes. This is a fast-paced position and productivity is important.Local travel into our office (Troy MI) may be required (Team Meetings or Trainings)Home office wit
