Job DescriptionThe Population Health RN Care Coordinator is a nurse care manager that functions within the multidisciplinary team as an advocate, educator and facilitator of resources for patients to ensure the appropriate access to evidence-based medical services that are patient-centered. The Population Health RN is: (1) an advocate: ensuring access to appropriate care and services, promoting self-care management and prevention; (2) a navigator: identifying and removing barriers to care and coordinating patient care services by utilizing the nursing process to help patients move through the healthcare continuum; (3) a collaborator: working within the multidisciplinary team to ensure a comprehensive plan of care in accordance with nationally recognized evidenc
