Shift: Day Shift- Monday – Thursday; 8:00am – 5:00pm and Friday; 8:00am – 12:00pm

The role you’ll contribute:
The Targeted Case Manager is responsible for conducting a comprehensive assessment and periodic reassessment of an individual’s strengths and needs and assist them in gaining access to identified medical, social, educational, and other needed services and support.

- Assist patients in obtaining needed referrals and support based on identified needs.
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
- Bachelor of arts or science degree in a behavioral science from an accredited university
- One year full-time employment working directly with individuals in a human service setting
- Knowledge of and ability to comply with state and federal statutes, rules and policies affecting the target population.
- Knowledge of available resources in the service area
Preferred qualifications:
- Master’s degree in behavioral science
- Completion or agree to complete AHCA-approved targeted case management training within three months of initially providing services
- Completion or agree to complete AHCA-approved targeted case management training within three months of initially providing services
