RN Case Manager
The RN Case Manager is responsible for the overall management of patient care for a small team of patients, living at home or in a facility. The CM sees the patient 1-2 times/week to monitor symptom control, disease progression, and patient status. The CM works with the Hospice team (which includes, Medical Director, Hospice attending, NP, SW, Chaplain, and home hospice aide to develop a plan of care along with the patient and their family. The CM is responsible for documenting the patient assessment in the electronic medical record and communicating findings with the family members and hospice attending.
