The UCSF Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco. We provide psychotherapy, substance-use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training. Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team. We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 40 years. Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision.
The Behavioral Health Case Manager position is part of the Behavioral Health Services team at the 1930 Market Street Service Center, as well as part of a collaboration with Zuckerberg San Francisco General Hospital (ZSFG) Positive Health Program (Ward 86). This role is responsible for providing case management services to individuals and groups who have Intravenous Drug Use (IVDU) and who are at-risk of HIV and other comorbid diseases, as part of the Health Access Point (HAP) for IVDU providing comprehensive HIV prevention and Harm Reduction services for these individuals. These clients are likely to have substance use disorders and potential co-occurring mental health disorders. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers, and psychologists.
This position provides case management services to individuals living with co-occurring diagnoses of substance use issues and severe mental illness, within a Harm Reduction philosophy. Motivational Interviewing, among other interventions, is utilized to help clients explore their relationship to their drug(s) of choice, address their ambivalent feelings about making changes to their use and creating client-centered, realistic and meaningful treatment plan goals for change. This position works under the clinical supervision of a licensed clinician and in conjunction with other teammates. This position will involve providing case management services consisting mainly of field work and home visits, escorting clients to appointments to assure engagement, linkage to substance use treatment and primary medical care services, assisting with retention in care, assessing clients' service needs and assisting clients with appropriate referrals, including for housing, vocational and other mental health services. Additionally, the Behavioral Health Case Manager will assist clients in creating and meeting treatment plan goals and reduce barriers to care and will provide supportive Harm Reduction-focused counseling services to clients, as needed. This position will involve co-location at ZSFG clinic site and working closely with the Ward 86 HAP and POP-UP (Positive-health Onsite Program for Unstably-housed Populations) Teams, which will provide medical and preventive care services to these at-risk clients, within a multidisciplinary team.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $37.56 - $50.31 (Hourly Rate).
