Responsibilities
The George Washington University Hospital (GW Hospital) is a 395-bed tertiary care, academic medical center located in downtown Washington, DC, next to the Foggy Bottom metro. POSITION SUMMARY
- The RN Care Transition Coordinator is a member of the Case Management Department responsible for the implementation and support of the GWUH's Care Transition Program targeted to improve continuity and quality of care while reducing the number of patients readmitted to the hospital within 30 days of discharge for non-Bundled Payment for Care Initiative-Advanced (BPCI-A) patients, and 90 days of discharge for all BPCI-A patients. Transition management will be provided to patients discharged to home, skilled nursing facilities, rehab and long term acute care facilities utilizing appropriate strategies for the specific level of care. The Care Transition Coordinator is a member of a multidisciplinary team including but not limited to the attending physician, nursing, pharmacists, case managers, nutritionists, rehab team, palliative care, Medical Faculty Associates primary care and specialists, and community partners. The Care Transition Coordinator meets with selected patients upon admission to the hospital and assists with their education, care coordination, transition to the appropriate next level of care and ongoing monitoring of their status after discharge from the hospital.

* BSN required
* Advanced Practice Degree - MSN or CNS preferred
* Current DC RN licensure
* 3-5 years of acute hospital experience required
* Inpatient Cardiac experience preferred.
* Case Management/Care Transition experience with a knowledge of case management principles and methodologies greatly preferred
* Knowledge of federal and state regulations preferred

* Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and in chronic disease management.
* Computer competency required.
