The Supervising Case Manager is part of the Behavioral Health Services (BHS) team at the 1930 Market Street Services Center, and is part of the clinical leadership of the BHS team. The Supervising Case Manager is responsible for providing administrative and clinical supervision to Social Work Associate and/or Clinical Social Worker staff who function as Case Managers; screens and assesses referred clients for service needs, psychosocial history and service plans; is responsible for contract compliance and meeting of contract deliverables, as well as carries a caseload of case management clients living with complex needs and co-occurring diagnoses of substance abuse issues, medical issues and severe mental illness. This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, psychologists, nurses, social workers, therapists, substance use counselors and case managers.
This position will partner heavily with the Getting to Zero Initiative as well as the ZSFG Positive Health Practice (Ward 86) provider team, providing supervision and guidance of case management services to those clinic HIV-positive patients primarily under the POP-UP and HAP Programs.
This position also provides counseling and case management services to individuals living with co-occurring diagnoses of substance abuse issues and severe mental illness, within a Harm Reduction philosophy, and utilizes Motivational Interviewing, among other interventions, to help clients explore their relationship to their drug(s) of choice, address their ambivalent feelings about making changes to their use and creating client-centered, realistic and meaningful treatment plan goals for change. This position works under the clinical supervision of a licensed mental health professional and in conjunction with other teammates. This position will involve providing intensive clinical case management services to clients who have been non-engaged in their medical care (particularly HIV medical care) and addressing the psychosocial barriers to clients engaging in, and maintaining, their treatment adherence. This position will involve field work and engaging clients in their home as well as community settings, and working collaboratively with collateral service providers to ensure continuity of care.
