Position Summary The LPN Case Manager supports safe and efficient patient transitions from hospital to home by combining clinical nursing skills with coordinated discharge planning. This role arranges home health services, home IV antibiotics, durable medical equipment, and transportation, while also addressing key social determinants of health such as housing, food access, and medication affordability. Working closely with RNs, social workers, providers, and community partners, the LPN Case Manager ensures patients receive the resources and support needed for optimal health.
Essential Duties
Coordinate home health services, including outpatient services, nursing, PT/OT/SLP, and supportive care.
Arrange home IV therapy, including communication with infusion companies and verification of discharge readiness.
Identify potential barriers to discharge and escalate concerns to the RN or provider.
Screen patients for housing instability, food insecurity, transportation barriers, and medication and co-pay assistance programs.
Connect patients and families with community resources, shelters, food banks, housing programs, Medicaid/financial assistance, and medication assistance programs.
Coordinate transportation through insurance benefits, rideshare partnerships, non-emergent medical transport, or community programs.
Serve as a liaison between the healthcare team, home health agencies, pharmacies, infusion companies, and community partners.
Participate in interdisciplinary rounds to update the team on patient needs, progress, and barriers.
Document coordination activities and resource referrals in Epic.
Ensure all discharge arrangements comply with CMS, Joint Commission, insurance, and hospital policies.
Participate in performance improvement projects and ongoing education related to case management and Social Determinants of Health.

Minimum Qualifications
Education: Licensed Professional Nurse (LPN) Required
Work Experience: 2 years experience as an LPN required. Case Management, discharge planning, and/or care coordination experience preferred.
Special Training, Certification or Licensure: Current Colorado LPN license required. Current CPR/BLS certification required.
