Clinical Case Manager
Clinical Case Manager Job Description: The clinical case manager supports the functioning of the Student Counseling Center by facilitating access to mental health care for students that is beyond the scope of services available on campus (i.e. facilitation of community mental health referrals for students needing long-term or specialized mental health care; coordination of care upon discharge from hospital when inpatient psychiatric care is warranted). The clinical case manager is involved in direct clinical service to students by participating in triage intakes, crisis intervention, and by providing intensive case management and transition services (bridge care) to facilitate stabilization until adequate community care plan is established.
