Required Qualifications (as evidenced by an attached resume):
For Social Workers: A Master of Social Work (MSW) degree (foreign equivalent or higher) with a specialization in clinical social work, clinical practice or mental health. Licensed in NY State as an LCSW or if licensed in another state they should be able to obtain licensure in NYS within one year of start date. Must have a minimum of 1 year of postgraduate clinical mental health work experience.
For Licensed Mental Health Counselors: A Master's degree (foreign equivalent or higher) in Mental Health Counseling with a minimum of 2 years clinical mental health work experience post licensure (and will obtain NY State licensure within one year of start date).
Experience providing clinical case management. Experience supervising professional staff.
Preferred Qualifications:
Five (5) years of work experience providing mental health services. Experience with clinical data management and reporting systems. Experience providing clinical or case management services in a university or college mental health setting. Experience providing risk assessment and crisis intervention in an outpatient treatment setting. Proficiency in spoken and/or written languages other than English.
Brief Description of Duties:
The Assistant Director of Case Management and Risk Mitigation oversees case management services, ensuring that students receive appropriate referrals, follow-up care, and seamless care transitions. This role involves consultation and collaboration with CAPS staff, campus partners, and external providers to manage complex clinical cases and high-risk clients to ensure timely follow-up, facilitate smooth care transitions, and provide comprehensive support. The role also provides direct clinical services, including crisis intervention, individual counseling, psychoeducation workshops, outreach, and other mental health services as needed. The Assistant Director provides leadership in developing policies and procedures for high-risk case management, suicide prevention, and addressing suicidality, offering guidance and training to staff on best practices in crisis response. Additionally, this role involves ensuring the maintenance of an accurate database, analyzing data, reviewing charts and conducting audits for accurate documentation, staying informed on national trends, and implementing best practices to enhance service delivery and improve student outcomes.
The incumbent plays a crucial role as a team member within the overall campus health and wellness departments. In this capacity, they collaborate with colleagues across various units to enhance the effectiveness and integration of health, wellness, and prevention services. This includes actively participating in divisional meetings, sharing insights, and contributing to collaborative decision-making processes. The incumbent fosters a cooperative and supportive environment, ensuring seamless teamwork and alignment with the shared goals and objectives of campus health and wellness initiatives. This position may be required to provide support in other areas of student health, wellness, and prevention services, as needed.
High-Risk Client Care Optimization:
- Create and maintain a database of high-risk clients active with CAPS treatment providers.
- Collaborate and consult with CAPS providers to monitor progress, mitigate risk, ensure smooth care transitions, and make connections to campus and community resources.
- Coordinate with the CAPS psychiatry providers to assess the scope of care for medication management for students transitioning after psychiatric hospitalization, ensuring adherence to best practices and reducing potential liabilities.
- Develop and manage day-to-day communication with outside care facilities (inpatient units, CPEP, etc.) on student care transitions, ensuring seamless coordination and risk mitigation strategies to minimize clinical or referral errors.
- Work with the University Police Department (UPD) to develop and maintain transport protocols for high-risk clients needing to access the Diagnostics Assessment and Stabilization Hub (DASH), Comprehensive Psychiatric Emergency Program (CPEP), or other external facilities, ensuring a safe and efficient response that adheres to regulatory and quality standards.
- Collaborate with the Student Support Team (SST), CARE, SASC, and other departments to assess the care needs of high-risk students to ensure that they are appropriately referred and treatment connected.
- Communicate effectively with CAPS leadership about emerging risks, mitigation strategies, and regulatory compliance.
Case Management Leadership and Coordination:
- Oversee case management services, ensuring students receive timely referrals, coordination of care, and follow-up support.
- Oversee the front desk process to ensure a positive and equitable client reception.
- Represent CAPS in the University's CARE team.
- Provide direct case management services to students, including those experiencing significant mental health challenges, crises, or complex care needs.
- Develop and maintain relationships with external mental health providers, hospitals, and other support services to facilitate smooth transitions for students in need of higher levels of care. Organize Annual Provider Breakfast to facilitate CAPS connection with community referral resources.
- Coordinate immediate crisis intervention for students walking into CAPS, ensuring rapid assessment and intervention to mitigate risks and prevent clinical or referral errors.
- Coordinate outreach to students marked as "urgent" by the CAPS Crisis Line, systematically identifying, assessing, and addressing risks in real time to prevent harm and ensure timely support.
Direct Clinical Service:
- Provide psychological services to students on both regular and emergency basis, and when necessary, during non-traditional hours. Services include but are not limited to, individual treatment, couple treatment, group treatment, crisis intervention, triage, clinical consultation, and case management.
- Maintain current, accurate clinical documentation to meet NY, federal (FERPA) and accreditation standards and CAPS policies in all areas. Serves all campuses including West, East (HSC), and Southampton campus locations and satellite offices.
Staff Training and Education:
- Provide ongoing training for clinical and administrative staff on risk management, client safety, and documentation best practices.
- Provide supervision to case managers, interns, and staff engaged in case management services.
- Assist in the development and implementation of policies, procedures, and best practices related to case management and high-risk client services (care transition, referral, etc.).
- Deliver workshops, trainings, and presentations for faculty, staff, and students on topics such as crisis response, mental health awareness, and navigating campus resources.
Professional Development and Committee Involvement:
- Participate in opportunities that promote your learning, growth, and development (i.e. classes, training, committee, webinar, etc.).
- Contribute to the Division of Student Affairs and Stony Brook University by attending and/or supporting events, activities, and programs sponsored by areas outside of your unit/department (i.e. campus-wide traditions, events, programs, etc.).
- Other duties or projects as assigned as appropriate to rank and departmental mission.
- Evening and weekend work may be required at times.
Special Notes:
This is a full-time appointment. FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.
In addition to the employee's base salary, this position is eligible for $3,400 UUP annual location pay, paid biweekly.
