Ambulatory Case Manager RNJob Summary The Case Manager will work independently but receives direction concerning the overall goals and objectives of the department and participates in actions to achieve these departmental goals. Performance of case management and care coordination is facilitated primarily through telephonic and electronic and/ or on-site communication with UCDH personnel and individual patients on a longitudinal, scheduled, or as needed frequency.
