Job Description Summary This position functions within the SEARHC Community Wellness Department, in the WISEWOMAN Women’s Health Programs and is responsible for coordinating the delivery of appropriate and timely care for patients, promoting quality and cost-effective health care outcomes. This position will work with women, ages 21+, by providing comprehensive care management for their breast and cervical cancer prevention and screenings, and cardiovascular health. All while working alongside the patient’s primary care provider’s care team for early detection and screening of breast and cervical cancers and cardiovascular disease, along with determining potential risk factors.
