Position Summary: Rutgers, The State University of New Jersey, is seeking a Health Care Case Manager for Mercer South for the Child Health Program within the School of Nursing. The Heath Care Case Manager (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office.Among the key duties of this position are the following:Upon entry into out-of-home placement, assists with enrollment of a child in a managed care entity and identification of a medical home (primary care provider),
