Case Manager (4260)
This position is responsible for medical case management related to the Ryan White Grants covering the Springfield and Decatur client population as assigned.Case Management 75%A. Function as a medical case manager, to include the following:1. Conduct client intake interview to determine eligibility for the program. The case manager must meet client where they are most comfortable and must be willing to complete home visits when necessary.2.
